Enrollment Information

To enroll a child in any one of our programs, please visit our Lake Ida campus located at 555 NW 4th Street in Delray Beach, Florida and bring the following:

• current, original physical and immunization record
• copy of the child’s birth certificate
• copy of child’s and parents’ social security cards
• list of emergency phone numbers
• doctor’s note for allergies or special medical concerns

 

An enrollment packet must be completed in person at the Achievement Centers. To inquire about subsidized
childcare funding, contact Family Central at (800) 683-3327.
